Phillip Ibrahim, a healthy 39-year-old man who has a habit of going to the gym and doesn`t smoke and drink, died suddenly in a big Sydney hospital.
One day in 2014, the father, a two-year-old, suffered from pneumonia, making it difficult to breathe. His wife rushed him to Concord Hospital in Sydney.
Ten days later, Mr. Ibrahim felt much better. He sat in the recliner and talked to his brother. However, just then, he suddenly fell down.
The hospital clerk rushed to his bed and found the lid of his infusion tube uncovered. This allows bubbles to enter his blood system, blocking his brain`s blood supply.
Mr. Ibrahim was transferred to Prince of Wales Hospital (Prince Of Wales Hospital), where he was found to have suffered severe brain damage. Two days later his life support equipment was shut down.
Philip`s father, Mohsin, says his son`s death left his two grandchildren without their father.
Today, the team investigating his death was told that doctors had ordered the infusion tube to be removed on the day of the air embolism, but the tube was still inserted because the nurse on duty was not authorized.
It was one of six similar deaths in New South Wales between 2012 and 2015, which the report said were avoidable.
Mr. Ibrahim`s widow and brother sat on court with his father, Mohsin Ibrahim, today, eager to get an answer, and hope that in the future the situation will be change.
Mohsin Ibrahim said: "preventable deaths cost a six-year-old boy and a two-year-old girl their father, leaving them without their father`s care, support or guidance all their lives."
"our politicians often tell us that we have a world-class health care system, but unfortunately, these avoidable deaths show that this is not the case."
He hopes the investigation will bring the necessary change to processes and regulations and laws to ensure that no more people lose their lives as a result.
